Expedite your initial consultation by downloading and filling out our intake paperwork ahead of time. Please fill out these forms completely and bring a copy to your first appointment. If you would like us to submit it to your insurance company, please bring your insurance card with you to your first appointment.

You can also choose to fill out the intake paperwork in our office prior to your initial consultation.  Please arrive 15 minutes before your scheduled appointment time so you will have adequate time to complete all paperwork.
