You can also choose to fill out the intake paperwork in our office prior to your initial consultation. Please arrive 15 minutes before your scheduled appointment time so you will have adequate time to complete all paperwork.
If you would like us to submit it to your insurance company, please bring your insurance card with you to your first appointment.
